Privacy Policy

Your health data never leaves your device. Apple bills your subscription, and Form never sees your payment details.

This policy covers what data the app reads, where it goes, what Form does with it, and what Form does not do.

What data does Form access?

Form reads health and workout data from Apple HealthKit on your iPhone or iPad. Specifically:

  • Workouts: type, duration, the heart rate samples recorded during each session, and the heart rate zones Apple calculated for it
  • Heart rate metrics: heart rate samples from the past 24 hours (used by Shadow Detection to spot activity you didn't log), resting heart rate, heart rate variability (SDNN), walking heart rate average, and 1-minute heart rate recovery
  • Cardio fitness: VO₂ max as estimated by Apple
  • Sleep: sleep analysis, including stages on watches that support them
  • Respiratory rate: overnight breathing rate (Apple Watch Series 8 and later), used to spot early signs of illness
  • Activity: steps, active energy burned, and walking speed
  • Body measurements: body mass, lean body mass, and height, used for weight trends and to estimate your calorie needs and protein target. Body mass and lean body mass are only present if you sync a scale to Apple Health or enter them there.
  • Nutrition: dietary protein, carbohydrates, fat, and energy, read only if a food-logging app writes them to Apple Health, so Form can show what you've eaten next to your targets
  • Profile characteristics: date of birth and biological sex, used only to compute heart rate zone boundaries and to label fitness norms (e.g., "above average for age 40–49 M")

You grant access through the standard iOS/iPadOS HealthKit permission prompt the first time you open the app. You can revoke any individual permission at any time from Settings → Privacy & Security → Health → Form on your iPhone or iPad, or from within the app at Settings → Health Permissions. If you grant only some permissions, the app still works, and tiles that depend on missing data are marked.

Form does not access your contacts, calendar, photos, location, microphone, camera, medical records, prescriptions, lab results, or any other apps installed on your device.

Form is read-only. It never writes data back to HealthKit.

Where does the data go?

Your health data stays on your iPhone or iPad, and Form processes it there.

  • We do not operate any servers for your health data.
  • There is no account to create.
  • Form does not sync anything to the cloud. Your Apple Health database itself may be backed up to your personal iCloud, depending on your iPhone or iPad's iCloud settings. That backup is Apple's, and Apple's privacy policy covers it. Form only reads from HealthKit on your device.
  • We do not transmit your HealthKit data to Apple, to us, or to any third party.

The only network requests Form makes are to Apple's StoreKit servers, to verify your subscription status (see Subscriptions and payments below). These requests contain none of your HealthKit data, only the anonymous transaction information Apple's StoreKit framework requires by default.

Notifications

Form can send four kinds of local notification: a morning nudge with today's recommendation, a recovery alert when your data shows a warning sign, a Sunday recap of your week (with a one-tap share action), and an occasional reminder if you haven't opened the app in a while. All four are built from the same on-device data described above. Form reads nothing extra to produce them and sends nothing about them anywhere.

  • Local only. Form has no push notification infrastructure and no backend. Every notification is scheduled on your iPhone or iPad using Apple's local notification APIs, and no server decides what to notify you about.
  • Permission. iOS asks for notification permission once, at the same moment it asks for HealthKit access. If you're updating from an older version that already has HealthKit access, you'll instead see a single one-time prompt focused on notifications only.
  • One toggle, all four types. A single "Notifications" switch in Settings turns all four types on or off. You can't configure them individually.
  • What's tracked to schedule them. Form keeps a small amount of on-device bookkeeping, such as the date you last opened the app, to decide when a nudge is due. It's stored the same way as the rest of Form's computed data (see below): locally, never transmitted, and deleted if you uninstall the app.

You can revoke notification permission at any time from Settings → Notifications → Form on your iPhone or iPad, or turn all four off from Form → Settings → Notifications.

Subscriptions and payments

Form is free for 30 days. After the trial, continued use requires a subscription, billed monthly ($2.99/mo) or annually ($29.99/yr).

All payments are processed by Apple via the App Store. Form never sees your credit card or any payment information.

What this means for your privacy:

  • We never see your credit card number, billing address, billing zip code, or any payment information. Apple handles all of that under their terms and the Apple Privacy Policy.
  • Form communicates with Apple's StoreKit servers on launch and occasionally in the background to verify your subscription is active. No HealthKit data, no personally identifying information, and no usage data is sent in these requests beyond what Apple's StoreKit framework includes by default.
  • We do not maintain a server to validate subscription receipts. If Apple says your subscription is active, the app trusts that.
  • Apple may share an anonymized subscription status indicator with us through App Store Connect's standard reporting (e.g., aggregated counts of active subscribers, trial conversions). We cannot link those aggregates to any individual user.

To manage or cancel your subscription, go to Settings → [Your Name] → Subscriptions on your iPhone or iPad. You keep access until the end of your current billing period. We don't make cancelling any harder than that.

Do you use analytics or tracking?

No. Form contains no analytics SDKs, tracking pixels, advertising identifiers, or third-party services of any kind. We can't tell who you are, how often you use the app, or what you do in it. The only data we see is the aggregated, anonymized reporting Apple provides for every App Store app, such as install and active subscriber counts.

Do you share or sell data?

No. We don't have your data, so there is nothing to share or sell.

What about derived data such as baselines, recent recommendations, and computed metrics?

Form computes baselines and rolling averages from your HealthKit data, for example your 60-day average HRV, your 30-day Zone 2 minutes, and your heart rate recovery over your last 4 sessions. These values are stored locally on your iPhone or iPad in the app's sandboxed storage and never leave the device. If you uninstall Form, iOS deletes this storage along with the app.

Nothing Form does changes your original HealthKit data. It stays in your Health app, controlled by Apple and covered by Apple's HealthKit privacy policy.

Children's privacy

Form is not designed for users under 13 and is not directed at children. We do not knowingly collect or process any data from users under 13. Because there are no accounts, if a child under 13 has access to Form, the best thing to do is uninstall the app from that device.

Changes to this policy

If we ever introduce features that change how data is handled, we will update this policy and the version number above before shipping the release that introduces the change. Material changes will be highlighted in the app's release notes on the App Store.

We don't have a mailing list. To keep track of material privacy changes, bookmark this page and check the version number at the top.

Your rights

Because Form does not collect or store your health data outside your device, you exercise the usual privacy rights (data access, deletion, portability) directly on your iPhone or iPad:

  • To see what Form has access to, open Settings → Health → Data Access & Devices → Form.
  • To delete data Form has computed, uninstall the app.
  • To restrict access, revoke specific HealthKit permissions in Settings.
  • To manage your subscription, go to Settings → [Your Name] → Subscriptions.

You don't need to email us for any of these. iOS gives you direct control.
